A Clinical Population Health Practice Improvement Manager demonstrates clinical expertise and effectively partners with internal team content leaders on clinical program development. Responsible for leading implementation of practice transformation training, development, and improvement of population health management for assigned customers. Provides effective partnerships for team-based care initiatives that are grounded in current evidenced based practice and are patient/family centered. Promotes a learning culture that fosters improved care, health promotion and sustainable value performance. Clinical Practice Improvement Managers are the senior faculty at quality improvement workshops and will be called to troubleshoot low performing clients, remove barriers and facilitate their success in implementing the programs. Serve as the lead in a team approach in guiding, coaching, and encouraging clinical practices to transition to a value based payment operation. This role works with Population Health Nurses to develop a practice workplan and conducts monthly meetings to review patient panels, provide training and consultation to increase the quality of patient care, reduce the cost of care, and improve patient outcomes.

Clinical Practice Improvement Manager Essential Duties and Responsibilities:
Provides practice transformation training/mentoring of participants including ongoing learning activities based on evolving program changes/identified needs.
Contributes knowledge and evolves content to achieve the triple aim.
Delivers educational content in a professional and competent manner in both live participant venues and virtual platforms.
Provides input into the building of patient care tools and documents that support a focused yet collaborative process for Care Coordination.
Effectively partners with clients in developing their Population Health roles and responsibilities
Proctors monthly clinical cohort calls and acts as the subject matter expert for client's clinical staff.
Utilizing professional practice and Practice Improvement Manager experience, provides innovative solutions for process improvement and program success.
Responsible for educating clients on the clinical and billing aspects of population health management.
Helps to ensure competency of client's Population Health Nurses.
Acts as a clinical expert on the use of data, including population health software, to improve quality and lower costs.
Integrates use of claims data to identify process improvement opportunities for better population health management and enhanced coordination of care.
Ensures ongoing alignment of Population Health Nurse efforts with Quality Measures specific to participant program.
Engages in professional development activities to effectively network toward improved professional practice and health policy advocacy.
Works closely with clients who are struggling to meet their objectives, removes barriers and escalates issues to the CMO and business leader.
Act as population health tool's Care Management super user.
Create training materials on population health tool's Care Management module.
Perform training on population health tool's Care Management Module and Analytic Module.
Assume accountability for the overall strategic approach to transformation for each client
Meet monthly with a cohort of practices as the lead contact for practice transformation.
Walk practices through Transformation content and assists with PDSA cycles.
Access solutions from subject matter experts on quality measures, reporting requirements, and processes and value-based payment policy and programs
Attend additional calls for assigned communities as needed, including but not limited to physician cohort calls, board meetings, quality reporting calls and remediation calls.
Lead practices in implementing a comprehensive plan for transformation
Provide client support to practices within their cohort around quality, data collection, reporting, and program initiatives
Conduct on-site meetings with practices as needed
Lead Quality Improvement Workshops
Host quarterly cohort calls with practices and Care Coordination Coaches
Coordinate with the AM in accomplishing the following:
Gather data from program participants using various data sources (public records, Company's Data Warehouse, engagement/attendance measures, and either direct telephone & email correspondence with the participant or through the relevant Project Administrator at the Company)
Tracks progress toward milestones and deadlines, and escalates concerns to manager in a timely manner
Works with Program participants to obtain missing data and establish reporting mechanism that works for the participant
Compiles report and agenda for monthly client meeting
Tracks and documents client progress toward program goals
Accountable for achieving practice transformation success metrics as defined by Caravan Health Leadership.
Competencies:To perform the job successfully, an individual should demonstrate the following.
Current licensure as a Registered Nurse with a B.S.N. or equivalent degree required. Advanced degree or equivalent experience preferred.
MPH or MHA preferred
Previous Care Coordination or Population Health Management experience preferred.
Advanced training or certification in clinical health coaching preferred.
Minimum of five years' clinical experience including managing chronic disease patients required.
Previous healthcare operations leadership experience preferred in the proximity of acute care delivery systems.
Demonstrates evidence of essential clinical leadership, communication, education, collaboration, and mentoring skills.
Demonstrates the ability to promote organizational learning through use of evidenced based practice.
Demonstrates the ability to promote innovation for effective process improvement.
Proficient in current communication technologies (email, webinars, cell phone, etc.).
Effective organizational skills, demonstrates ability to maintain accurate notes and records.
Previous experience with health IT systems and data reports preferred.
Knowledge and sensitivity of various communication strategies, and ability to identify appropriate communication strategies for various settings.
Ability and willingness to travel up to 40% of the time and to rural settings.
At ease working in a decentralized "home office" environment of a virtual consulting management firm.
Ability and comfort to work independently with minimal direct supervision.
Experience with and passion for health care quality improvement and transformation.
2+ years' experience in provider clinic or other health setting preferred, experience working with clinical practice managers and care coordinators highly valued.
8+ years experience in provider clinic or other health setting preferred.
A team player; ability to work closely with a team to support client needs.
Experience working with electronic health record systems required.
Strong project management skills preferred.
Self-motivation and initiative in learning new programs, policies and skills.
Critical thinking skills necessary to develop a comprehensive practice improvement plan.
Ability to work in a high-case load, fast-paced environment with rapidly changing priorities, flexibility and a positive attitude.
Strong oral and written communication skills.
Ability to cultivate strong professional client-service rapport.
Detail-oriented; focuses on operational progress, program effectiveness and results.
Demonstrated experience with Microsoft Office Suite and population health software.
Certificates and Licenses:
Advanced degree preferred.
Clinician (RN, BSN) licensure preferred.
Certified Professional in Healthcare Quality preferred
