We have an outstanding opportunity to work for a fast-growing healthcare technology consulting organization, and are seeking a health equity-focused Population Health Technology Trainer with recent, relevant, successful experience training staff and clients to utilize and interpret population health data to better understand total cost, utilization and quality of care for a defined patient population (typically part of an Accountable Care Organization (ACO) or similar environment).

·     RN, NP, PA or other clinical degree from an accredited college or University.


·     2-3 years' experience in health informatics, health administration, or related field.


·     Experience using technology to facilitate clinical workflows and building templates.


·     2-3 years of previous data analysis and/or applied research experience.

 Previous HMO/Managed Care or Healthcare industry experience strongly preferred; experience or interest in understanding patient care delivery in rural health care setting a plus.

·     Preferred understanding of health data formats including: Medicare claims, EMR, lab and pharmacy.

·     Excellent knowledge of data collection, analysis and data presentation.

·     Prefer industry knowledge including Accountable care organizations, health care reform, population health management models.

·     Experience with quality reporting such as HEDIS, PQRS or CMS STAR ratings, highly preferred.

·     Experience using statistical packages for analyzing large data sets a plus.

·     Excellent oral and written communication skills; ability to present complex information in an understandable and compelling manner.

·     Excellent critical thinking and problem-solving skills.

·     Strong interpersonal, communication and collaboration skills.

  • Demonstrated ability to complete projects in an environment with diverse, competing and sometimes conflicting priorities.
  • Ability to plan, develop, implement, and evaluate participant performance against risk-based population health programs and develop tactics to improve performance and minimize total cost of care.
  • Ability to present concepts and analysis in front of diverse audiences.
  • Interest in contributing to a fast paced and growing company 


Additional Responsibilities


·     Design and lead the Population Health Management System training program.

·     Become an expert "superuser" of Lightbeam Heath Solutions, a data warehouse and analytics system, and support internal and external stakeholders in optimizing their use of the Lightbeam population management system.

·     Create and upload relevant education videos in the learning management system; develop associated written education documents to accompany training videos.

·     Facilitate Monthly User Group Meetings.

·     Create weekly "tips and tricks" article to post in newsletter.

·     Translate clinical workflows into processes in Lightbeam.

·     Travel to perform on-site training as requested by clients.

·     Train internal client-facing staff.

·     Create short training videos.

·     Increase one-on-one training access to clients.

·     Host in-person trainings.

·     Create "Super-User" tests and "certification" courses to establish fundamental level of comprehension.

·     Utilize Lightbeam utilization data (aggregate data from survey & SQL DB including LB usage).

·     Develop and champion the use of "train the trainer" material – instructing clients how they can teach other staff in their organizations who will use Lightbeam.

·     Create and maintain clinical administrative functions in Lightbeam

o   Cohort Builder

o   Care plan Builder

·     Lead the quality assurance of new software version releases in the Care Management module and cohorts.

·     Learn and adhere to HIPAA Compliance as it relates to the role and department.
