Position Summary

 

The Clinical Team Leader ensures standardization and alignment of the clinical leaders' clinical coaching activities as well as facilitates the development of training methodology aimed at successfully transitioning customers to value based care.  Maintains a focus on leveraging current and evidenced based population health practices. The Team Leader is also responsible for rallying the individual Clinical Leaders to assist the Company's SMEs with content development and content editing.  This person serves as a Clinical Team relationship manager and escalation point relating to the performance of the clinical leaders. The Team Leader encourages effective consistent communication from all members of the Clinical Leader Team and promotes a learning culture fostering improved health care, lower costs and sustainability. The Team Leader closely collaborates with and reports directly to the Vice President of Customer Success in ensuring targeted results are achieved.

 

As a Clinical Leader individual demonstrates clinical expertise and effectively partners with internal team content leaders on clinical program development. Responsible for leading implementation of practice transformation training, development, and improvement of population health management for assigned customers. Provides effective partnerships for team-based care initiatives that are grounded in current evidenced based practice and are patient/family centered.  Promotes a learning culture that fosters improved care, health promotion and sustainable value performance.  Each clinical leader will have a team of Practice Improvement Managers and assigned accounts, and will act as the subject matter expert for that group of clients, providing knowledge and direction and take ultimate accountability for the performance of their customers. Clinical Leaders are the senior faculty on workshops and will be called to troubleshoot low performing clients, remove barriers and facilitate their success in implementing the programs.

 Clinical Team Leader Essential Duties and Responsibilities:

  • Facilitate the development and continued sense of Team among the members of the Clinical Team  
  • Optimize each member's ability to effectively and efficiently train our customers in established evidence based practice and person/family centeredness
  • Promote effective training by Program Coaches of member cohort teams.  Participate and assist in leading ongoing learning activities based on evolving program changes/identified needs.
  • Help ensure the competency and effectiveness of the clinical leaders in delivering content to customers through workshops, cohort calls and Steering Committee activities
  • Facilitate a minimum monthly Clinical Leader Team Conference Call with the Vice President of Customer Success
  • Attend scheduled 1:1 calls with VP of Customer Success
  • Partner with VP of Customer Success regarding any identified Clinical Leader performance issue and formulate recommended strategies for performance improvement
  • Partner with the Company's SMEs to build program subject matter expertise within the Clinical Leader Team
  • Ensure consistent delivery of messaging to customers by Clinical Leaders
  • Promote and encourage active participating by the Clinical Leaders in the "Triad" Team meetings to ensure customers are served effectively
  • Serve as the contact-point, advocate, and informational resource for NRACO Coach team

Clinical Leader Essential Duties and Responsibilities:

  • Provides practice transformation training/mentoring of participants including ongoing learning activities based on evolving program changes/identified needs.
  • Contributes knowledge and evolves content to achieve the triple aim.
  • Delivers educational content in a professional and competent manner in both live participant venues and virtual platforms.
  • Provides input into the building of patient care tools and documents that support a focused yet collaborative process for Care Coordination.
  • Effectively partners with clients in developing their Population Health roles and responsibilities
  • Assists PIMs and clients in overcoming barriers to success.
  • Proctors monthly clinical cohort calls and acts as the subject matter expert for client's clinical staff.
  • Utilizing professional practice and clinical leadership experience, provides innovative solutions for process improvement and program success.
  • Responsible for educating clients on the clinical and billing aspects of population health management. Ensures competency of client's Population Health Nurses. 
  • In collaboration with the Practice Improvement Manager, is responsible for supporting client success toward achieving practice improvement outcomes.
  • Acts as a clinical expert on the use of data, including population health software, to improve quality and lower costs.
  • Integrates use of claims data to identify process improvement opportunities for better population health management and enhanced coordination of care.
  • Ensures ongoing alignment of Population Health Nurse efforts with Quality Measures specific to participant program.
  • Engages in professional development activities to effectively network toward improved professional practice and health policy advocacy.
  • Works closely with clients who are struggling to meet their objectives, removes barriers and escalates issues to the CMO and business leader.
  • Act as population health tool's Care Management super user.
  • Create training materials on population health tool's Care Management module.
  • Perform training on population health tool's Care Management module and Analytic Module.
  • Travel up to 40%

      • Clinical Team Leader Role Experience Requirements:

         

         

        • Previous leadership experience required, experience in hospital and/or ambulatory operations management.
        • Demonstrates evidence of essential leadership, communication, education, and collaboration.
        • Previous Care Coordination or Population Health Management experience.
        • Demonstrates the ability to promote individual learning and professional development of staff.
        • Demonstrates the ability to promote innovation for effective process improvement.

         

         

        Clinical Leader Education/Experience Required:

         

        • Current licensure as a Registered Nurse with a B.S.N. or equivalent degree required.
        • Minimum of five years' clinical experience including managing chronic disease patients.
        • Previous healthcare operations leadership in the proximity of acute care delivery systems
        • Demonstrates evidence of essential clinical leadership, communication, education, collaboration, and mentoring skills.
        • Demonstrates the ability to promote organizational learning through use of evidenced based practice.
        • Demonstrates the ability to promote innovation for effective process improvement.
        • Proficient in current communication technologies (email, webinars, cell phone, etc.).
        • Effective organizational skills, demonstrates ability to maintain accurate notes and records.
        • Knowledge and sensitivity of various communication strategies, and ability to identify appropriate communication strategies for various settings.
        • Ability and willingness to travel up to 40% of the time to rural settings.
        • At ease working in a decentralized "home office" environment of a virtual consulting management firm.
        • Ability and comfort to work independently with minimal direct supervision.
        • Education/Experience Preferred:

           

          • Previous Care Coordination or Population Health Management experience.
          • Advanced training or certification in clinical health coaching.
          • Previous experience with health IT systems and data reports.
          • Advanced degree.

           

          Travel Requirements:

           Not to exceed 40%, with seasonal peak activities
